Just released: One more publication of special issue “Lifestyle, Diet, Wine and Health”
Following the first two “Lifestyle, Diet, Wine and Health” congresses, in Toledo and Rome, a peer-reviewed special issue in Nutrients is being compiled.

Two of the authors are WIC experts: Dr. Creina Stockley and Prof. Mladen Boban, as well as the distinguished Prof. Curtis Ellison (physician, epidemiologist, and emeritus professor of medicine at Boston University, USA).
Interpreting the Evidence on Wine Consumption and Health: A Narrative Review of the J-Shaped Association and Contemporary Alcohol Guidelines
The relationship between the consumption of alcoholic beverages and health has long been the focus of epidemiology, nutritional science, and public health discourse. Among alcoholic beverages, wine has attracted particular attention due to its polyphenolic content with antioxidant and anti-inflammatory properties, contributing to cardiovascular benefits. The existing evidence supports a J-shaped association, where light-to-moderate wine consumption is linked to lower mortality risk compared to abstaining or heavy drinking, yet recent public health guidelines in several countries have become more restrictive, suggesting there is no safe level of alcohol. This shift contrasts with previous guidelines that acknowledged the benefits of moderate wine consumption.
The authors emphasize the need for a nuanced understanding of wine’s specific health effects, as many reviews combine different types of alcoholic beverages without distinguishing their unique health impacts. A critical examination of how recent methodological advances in research and guideline development have influenced the interpretation of evidence related to wine and health, is needed.
The article describes the impact of drinking patterns on health outcomes:
- Moderate vs. Heavy Drinking: Moderate wine consumption is associated with favourable health outcomes, particularly cardiovascular benefits, while heavy or irregular drinking can lead to negative health effects, including increased mortality and higher risks of diseases like cancer and liver disease.
- Drinking Context: The context in which wine/alcohol is consumed (e.g., with meals, as part of a Mediterranean diet) influences health outcomes. Drinking wine with meals, for example, may enhance its protective benefits due to complementary dietary factors.
- Age and Baseline Health: The health implications of drinking patterns can vary by age and individual health status. Middle-aged and older adults may experience more significant benefits from moderate consumption, while younger individuals might face higher risks from alcohol-related harms.
- Episodic Heavy Drinking vs. Regular Moderate Intake: Guidelines often treat all alcoholic beverages consumption as homogeneous, but episodic heavy drinking is distinct from regular moderate drinking, which has different biological and health consequences. This differentiation is crucial for accurate health assessments and recommendations.

Overall, the evidence suggests that moderate, regular wine consumption with meals may be compatible with a healthy dietary pattern for some adults, particularly in mid- to later life, although this relationship is unlikely to be uniform across populations. Future research should increasingly adopt precision nutrition and precision prevention approaches, integrating prospective epidemiological, clinical, dietary, biomarker, and genetic data to identify population subgroups with distinct risk–benefit profiles.
